Our Mortgage Application Process

These forms will aid you in providing us with important mortgage information needed to process any loans. These are the initial forms required and will save you time by allowing you to print them at home, fill them out, and return them to one of our banking locations.

Bank of Pontiac Mortgage Loan Originators are included in the Nationwide Mortgage Licensing System & Registry (NMLS).  Their individual NMLS numbers are available upon request.


We recommend that you download the entire Mortgage Packet. This will save you considerable time rather than selecting the individual forms. The individual forms are available for users with slower internet connections.
